Job Overview
Job Purpose:
The Health Visitor will provide expert clinical advice, supervision and support to the health visiting skill mix team around Public health matters. The Health Visitor will work as part of the quadrant integrated team to support the Integrated team Managers and Clinical Team Leaders in ensuring that each member of the health visiting team is competent, able to meet the full needs of clients and operates to high standards. You will be supporting the Clinical Team Leaders with day to day operational management and team leadership to ensure effective delivery of the community health services in partnership with key partners and stakeholders.
Main Responsibilities:
- To undertake comprehensive initial assessments and screening using the appropriate evidence based tools and equipment and key stages along the 0-5 years life course.
- To facilitate and co-ordinate health promotion programmes in partnership with statutory and voluntary agencies within established groups.
- Preparing reports for and participate in in child protection conferences, core group meetings, court attendances and multi-agency planning meetings as required.
- Develop and maintain positive, effective and timely liaison and communications with all appropriate staff, professionals, parents, carers and agencies to monitor and ensure that actions in relation to individual care plans are implemented.
